Entry Requirements:
1. Resume/CV with duties and responsibilities
2. Transcript/Diploma/Cert/PRC
3. Certificate Of Employment with minimum experience
4. Certificate of Training Related to Pharmacy Practice
5. Self-Assessment Guide
6. 4pcs of passport size picture with name printed, white background.
7. Application Form
8. Clear Folder (Long)

Assessment Dates:
Every Wednesday & Friday
August 7, 9, 14, 16, 21, 23, 28 & 30

Note: Submission of requirements should be made one week before the scheduled assessment
